You may fill form and send it over the internet to us so that we may prepare your order for shipping in advance. Or, you may print out the form and mail it to us with your check or money order. We do request, however, that we receive your check or money order (in U.S. dollars, please) before we send your items to you (50% down on all items $300 or higher, and full payment on $299 or less). All prices include tax, shipping within the continental United States, and handling. International shipping charges may cost extra, please inquire before making your order.
